I haven't played since a few months, but when wandering around, look for those tall, white, smiling things. There is a normal one, and there is a slightly bloody one. A few steps northwest from the bloody one, there will be a creature which is just a floating head, with some other things below (if I recall correctly), kinda forming a gate. Pass through this "gate", then keep going north.
