"Key" does not count as an effect (it doesn't increase the player's level, which serves as the effect counter). "Instructions" does. It's not one of the 35 effects needed to trigger the ending.
You may have lost an effect when upgrading from an older version. The versioning script will sometimes confiscate certain effects depending on which version of the game you upgraded from.
