The main area of this world is a looping expanse punctuated by structures composed of large, rusty metal blocks. Many of these structures feature ladders on the front, allowing Minnatsuki to climb them. The occasional metal block features etchings of heart symbols upon its surface.
Objects that resemble three-dimensional heart symbols feature prominently throughout the area. These objects come in both pink and red varieties, as well as varying considerably in size. Some feature cracked surfaces, and are wrapped in bandages which partially cover the fractures. Some of the smaller heart symbol-shaped objects are contained within oversized glass jars.
Walking between two white rectangular pillars will transport Minnatsuki to Flesh Walls Forest. This connection, however, is initially closed off by an electrical barrier of red zigzagging lines. In order to enter Flesh Walls Forest for the first time, the Life Support event must be activated by interacting with a life support machine found southwest of the two pillars, causing it to become unplugged from the wall and the electrogram on its monitor to flatline. This will disable the barrier for the duration of the event. When the Life Support event is activated, the screen will momentarily fade to black before all of the heart symbol-shaped objects throughout the world will transform into anatomically accurate hearts. Additionally, the area's background music will be replaced with the sound of a heartbeat, which will become quicker over time. Furthermore, not only will the initially friendly heart-shaped NPC become aggressive and chase Minnatsuki down, but additional copies of it—also in an aggressive state—will appear throughout the area, moving progressively faster as time elapses. If caught, Minnatsuki will be kicked out of the main area of Heart World and must re-enter. After successfully passing through the two pillars and reaching Flesh Walls Forest for the first time, the electrical barrier between the pillars will disappear permanently, and on repeat visits it will no longer be required to activate the Life Support event in order to access Flesh Walls Forest.
