That item with the stars is debug mode, basically.
edit: stars might not show up on your system. The first item on the items list (item 0001) is what pulls up the game's custom debug panel when you enable debug mode using the trigger hidden in the wall in Urotsuki's room. It's also often used as a condition for stuff that's still being worked on.
