I don't know how to unlock it exactly, (It didn't appear when I tested it on a New Game) but it seems that in Version 0.100, when you wake up on the balcony, the step-ladder is there!
When you jump off of it... Well, I'll let you find out for yourselves. I think It supports the Dissociative Identity Disorder theory pretty well.
