Oil Puddle World B is an area accessible from the mushroom gate in Mushroom World or a gate in Heart World, both of which only appear after the player has viewed Ending #1.

Entering from Mushroom World or Heart World places you in a small, loosely spiral shaped maze made of invisible walls with static in the background. In the center is a black and grey door, which leads to the main area of Oil Puddle World B.
Oil Puddle World B is a gloomy, spacious void of shiny puddles. Strange differently colored creatures are found all across this world. After entering from either Mushroom World or Heart World, you will be placed in front of a door. Going through this door leads to you The Nexus in a one-way. East and slightly south of the entrance is a brown gate that leads you to Ethnic World.
