Star River consists of a winding structure of platforms and bridges over a starry sea. Most of the sea isn't traversible, but select locations will allow you to step down into its waters and wander a predetermined path. Glowing ball chasers inhabit the area, which will take you to an inaccessible area if touched - though they are slow, so they're easy to avoid as long as you don't let yourself get trapped with one tracking you. This area is the location of the Frozen Sparrowhawk delusion.
In the biggest chunk of water you can walk around in, you will find a headlike structure in the water. Walking into this structure will lead you to a room with a large, doglike creature chained to the wall. It seemingly does nothing when interacted with, unless you use the Thunder Ghost on it, resulting in its death and subsequent transformation into a one-way connection to the Dungeon.
