This page lists all the Delusions(effects) for Qiu Qin, what they do, and where to locate them.
Along with the regular 22 Delusions, you start off with an item called Default that returns Qiu Qin to her regular form, also taking off any pseudo-effects.

Action: The appendage on top of Qiu Qin's head grows and flashes, playing meowing sound effects and causing it to snow.

Appearance: Qiu Qin wears a golden armor.
Passive Effect: None.
Action: Qiu Qin lifts the visor on her helmet.
Location: Golden Canopy

Spear of Fear

Appearance: Qiu Qin's head turns into a rusty spear, shirt changes to red.
Passive Effect: Unlocks the ability to save at certain spots in the dream world while the Delusion is equipped.
Action: Qiu Qin's torso starts melting away as a strange sound effect plays.

Practical Uses: Extremely valuable tool that can help you get through areas like Hellish Prison and Hellish Trial without fear of needing to backtrack if you mess up.

Appearance: Qiu Qin's head turns into a bottle of poison.

Action: A sound effect is played as the bottle of poison springs a leak.

Action: A shadowy blue head pops up behind Qiu Qin and makes a noise.

Slayer
Spacesuit Delusion is required for the first location.

Appearance: Qiu Qin becomes a blue child with horns.
Passive Effect: None.
Action: Knifed tentacles burst from Qiu Qins chest and stab the space in front of her.
Location: The Hell or Freezing Point
Practical Uses: Allows you to kill certain NPCs, netting you money and materials that you can use to buy Miracle Potions or Zabu's items at the store in Red Aisle. Slayer is also required in order to obtain the Flying Machine and Multi Transformation Delusions.
Virus
"- A ghostly germ."
Appearance: Qiu Qin becomes a purple virus.
Passive Effect: Allows Qiu Qin to jump off the cliff in Building.
Action: Qiu Qin sprouts light purple wings and releases a black substance from her mouth.
Location: Star of Wise Sea A
Practical Uses: Required in order to obtain the Imprint Amulet Delusion.
Imprint Amulet
Virus Delusion is required.

Appearance: Qiu Qin's head turns into a pair of black insect legs.

Flying Machine

Appearance: Qiu Qin wears a pair of rocket shoes.
Passive Effect: None.
Action: Qiu Qin loses her balance and struggles in the air for a bit.
Location: Cloudy Sunset
Practical Uses: Allows Qiu Qin to fly over gaps and access otherwise unreachable areas in places like Cloudy Sunset, Floating Cube RPG, and the Elevator in The Hell.
Strange Egg
"- Back to familiar territory."
Appearance: Qiu Qin wears a cracked egg shell.
Passive Effect: None.
Action: The cracked egg rapidly reforms around Qiu Qin, returning her to Unreal Nexus.
Location: Hellish Park
Practical Uses: Allows you to teleport back to Unreal Nexus whenever you want without having to wake up.
Multi Transformation
Slayer Delusion is required.

Appearance: Qiu Qin turns into a floating slime.
Passive Effect: Allows Qiu Qin to transform into most NPCs in the game by interacting with them. The effects of the transformation are lost after unequipping the Delusion however.

Passive Effect: Provides Qiu Qin with oxygen in Space Station.

Thunder Ghost

Appearance: Qiu Qin becomes an Oni, wielding a wooden club. There is a small chance that a close up of the thunder ghost's face will replace the normal delusion sprite in the menu.
Passive Effect: Qiu Qin will swing her club at NPCs, killing certain ones that the Slayer Delusion cannot.
Action: Qiu Qin summons a fireball in her right hand.
Location: Pink Cube Zone
Practical Uses: Needed to access Dungeon from the Dog House in Star River for the first time.
Frozen Sparrowhawk
"- Summon a thunderstorm."
Appearance: Qiu Qin becomes a blue electric humanoid sparrowhawk. She sparks with electricity when she moves.
Passive Effect: None.
Action: Qiu Qin sparks with electricity, summoning rain.
